To obtain reprints at the special author rate, they must be ordered in advance. An order form showing the cost of reprints is sent directly from the publisher with page proofs. Reprint orders, payments, and inquiries should be directed to

Processing Fee

A non-refundable processing fee of $30 is required for each article, Short Communication, or uninvited Review Article. A check or money order should accompany each manuscript before it can be considered for review.
